1. Do you have any Medicaid along with your Medicare? (Must be

2. Are you enrolled in a supplement or advantage program, in
addition to Medicare Parts A & B? (Must Be Yes)
3. Are you currently making more than $1300 monthly or more
than $2700 monthly if you are married? (Must Be No)
